This role will include working with Primary Care colleagues, patients and carers to navigate the voluntary/ community services environment to address the wider detriments of health and support social and emotional well-being. Developing effective relationships with providers of voluntary and community services and maintaining a good knowledge of various programmes offered locally, aimed at meeting individualised needs. To also maintain regular contact with some patients and carers, acting as the first point of contact if needs change or escalate.
Main Duties and Responsibilities (This is not an exhaustive list):
· To provide personalised support and follow up for patients
· Build trusted relationships and work in a person-centred way
· To manage and maintain own caseload and diary
· Support of patients may be by phone, letter, email, face to face at the patient’s home, in the community or within their GP surgery.
· Provide information regarding services available and signpost appropriately.
· Assist with and develop the network patient peer support and social groups including coffee mornings, and specific support groups.
· To accurately record all patient contact across a range of electronic systems, and use this to effectively to manage caseloads and appointment scheduling
· To develop networks with partner organisations.
· To seek and record patient feedback to understand whether needs are being met, and to plan any necessary adjustments.
· To undertake any other duties or projects commensurate with the nature and grade of this post as required.
· To work in a manner that facilitates inclusion, particularly of patients identified as frail and vulnerable.
· To administer and organise own work to ensure that it is accurate and meets targets, reasonable deadlines and reporting requirements
· To adhere to all employing Practice and Network, policies and procedures and Primary Care standards, policies and procedures as defined by your Line Manager.
· To undertake relevant training in health and safety policy and procedures, ensuring that all practices and procedures are undertaken in accordance with a healthy and safe working environment.
· To be responsible for personal learning and development.
· Ensuring completion on PCN mandatory training on Teamnet.
Person Specification (This is not an exhaustive list):
· NVQ Level 3 or equivalent experience.
· Willingness to complete study/ training as appropriate to the role.
· Experience of working in a Person-centred way and through 1:1 support work
· Interagency working
· Understanding of voluntary sector services.

· Effective organisational skills, ability to prioritise.
· An understanding of people with mental health needs and accepting of other people’s lifestyles

· Awareness of Health & Social Care Policy
· Experience of the housing system and housing applications
· Understanding of the effects of homelessness or potential homelessness on clients
· Experience of supporting people with debt
· A good understanding of issues facing carers and those living with chronic disease.

· Pharmacy Team made up of a number of experience Clinical Pharmacists and Pharmacy Technicians supporting our practices and patients
· First Contact Physiotherapy Practitioner service
· Digital Care Coordinator, Digital Champions
· Health & Wellbeing Coaches within Help & Care team
