Woodside Health and Care Centre

NHS Greater Glasgow and Clyde (NHSGGC) is one of the largest healthcare systems in the United Kingdom, employing approximately 40,000 staff across a wide range of clinical and non-clinical professions and roles. We deliver acute hospital, primary, community and mental health services to a population of over 1.15 million people, and to a wider population of 2.2 million when regional and national services are included.

The role:

The Community Respiratory Team is a dynamic and responsive multidisciplinary team (MDT) that aims to support patients with moderate to severe COPD within Glasgow City in their own homes.

The team is comprised of Physiotherapists, Nurses, OT, Pharmacist, Rehabilitation Support Worker, Business Support and Leadership roles.

It is an evidence based service with the MDT supporting three distinct intervention pathways:

Prevention of hospital admission (support for acute exacerbation)

Early supported discharge

Chronic disease management (rehabilitation and self-management)

The post holder will work as a specialist respiratory nurse working within the oncall team. In addition they will support more junior staff with supervision and TURAS.

The post holder will act independently and use their own initiative within the bounds of existing knowledge and skills. They will work autonomously within their scope of practice to make clinical judgements regarding patient prioritisation and management.

Work is triaged and allocating depending on clinical patients need. You will link with the Respiratory Early Supported Discharge (ESD) team, Pulmonary Rehab team and Consultants at our MDT. You should have a nursing degree and have current NMC registration and experience working with COPD or respiratory patients.

This post requires membership of Disclosure Scotland's PVG

Duration, Location, and Working Pattern:

1. There is a full-time post (36 hours per week) and part-time post (22 hours per week) available.

Please note that the salary will be Pro Rata for part-time posts.

2. The shift pattern for this post is Monday - Friday 8.30am -4.30pm, with potential for weekend work in the future.
3. You will be based within the Woodside Health and Care Centre.

Key responsibilities:

Clinical/Specialist Knowledge

4. Undertake and provide clinical leadership assessment of patient needs, implementation and evaluation of programmes of care, which are evidence-based. Utilising all available resources taking into consideration the lifestyle, gender and cultural background, and ensure involvement with the patient, family, carers and significant others.
5. Act as a specialist resource for the clinical management of patients.
6. Act as a role model through the provision of professional leadership and demonstration of competent and effective practice.
7. Act as effective change agent, integrating information gained from research and audit into clinical practice.
8. Maintain patient records in line with NMC guidelines for records and record keeping.

Knowledge, training, qualifications and/or experience required to do the job:

Essential

9. First Level Registered Nurse with relevant post registration experience.
10. Respiratory or relevant clinical experience.
11. As appropriate to area of clinical practice demonstrate the ability to complete the agreed training pathway to develop CNS role.
12. Ability to fulfil criteria outline in the job description.
13. To have experience of working with patient /user/carer groups, or the ability to relate to patient/user/carers.
14. To have skills to communicate learning on an interpersonal level and in presentations.
15. A flexible approach to the role, in order to be able to manage working with a national programme, whilst delivering a service in a local setting.
16. Willing to travel to regional and national events
