Supervise the provision of care and support on site at an Extra Care Scheme, ensuring staff undertake duties with sensitivity to preserve dignity, privacy, choice, independence, and rights of Service Users and their families. Deputise at other schemes in the absence of Senior Support Workers.
Duties and Responsibilities
1. Maintain effective communication and relationships with staff and customers.
2. Liaise with health, social care professionals, and agencies involved in customer support.
3. Adhere to all policies and procedures.
4. Alert management to changes in Service User's condition.
5. Help manage staff rota weekly, ensuring sufficient staffing.
6. Participate in Needs Assessments, Risk Assessments, and Support Planning.
7. Deliver outcome-based reablement care and support.
8. Monitor progress towards outcomes and goals.
9. Conduct risk assessments, spot checks, staff reviews, and PDRs.
10. Implement an outcome-focused, reabling ethos in daily care.
11. Support housing-related tasks such as welfare benefits advice, signposting, health maintenance, and independence promotion.
12. Support a customer/residents committee for activities and events.
13. Ensure a safe living environment respecting Service Users' rights and choices.
14. Protect Service Users from abuse and ensure activities do not put them at risk.
15. Work independently and as part of a team.
16. Attend supervision, training, and meetings.
17. Assist with recruitment, literacy assessments, and interviews as needed.
18. Audit care documentation including MAR charts, logs, and quality records.
