Due to re-organisation and restructure in the integrated service, we have Band 6 opportunities for Physiotherapists
Working to a home first agenda, the role is to support service delivery across inpatient, emergency department and community settings supporting our work to enhance our Discharge to Assess community model.
Our service consists of Health and Social Care colleagues including Consultants, GPs, Advanced Care Practitioners, Social Workers, Nursing, OT, Physiotherapists, Discharge Team and Multi-Disciplinary Assistants all work together to support patient flow. We operate 7 days, 365 days a year to support some of our most vulnerable, frail patients.
You will work across the Harrogate and Rural footprint addition to our new purpose-built Hub at Harrogate Hospital, working with, colleagues from social care and our Acute Therapy Team.
Main duties of the job
* To carry a variable clinical caseload of patients within the Acute Response and Rehabilitation in the Community and Hospital Service (ARCH) providing complex physiotherapy assessments addressing physiotherapy performance, skills deficits, self-maintenance and cognitive function.
* To provide assessments and interventions which support an ‘admission avoidance’ or ‘home first’ approach enabling patients to receive care in their usual place of residence wherever possible
* To provide rehabilitation and advice to patients either at home or in hospital/rehabilitation bed to enable them to achieve their optimum level of independence and quality of life
* To work with patients, carers, relatives and colleagues to identify, plan, develop and implement Physiotherapy patient goals as part of an overall care plan.
* To plan and implement individual interventions, in collaboration with the patient, using graded activity to achieve therapeutic goals.
* To work independently under the guidance of the senior staff within the team and actively participate in the professional development of self and others.
* To advise on, and prescribe, specialist, assistive equipment and adaptations for the home.
* To take a responsible place in the multi-disciplinary team promoting the role of Physiotherapy and taking an active part in team projects.
* To support the collection of data and production of reports to demonstrate the value of the service and support recommendations for future expansion.

Essential criteria
* BSc (Hons) Physiotherapy or equivalent
* Completed a range of post-grad training on relevant subjects
* Minimum 18 months post graduate experience
* Experience of acute wards and or community work
* Skilled in a wide range of physiotherapy assessments in a variety of settings
Desirable criteria
* Evidence of supporting the delivery of service development
* Completed or working towards professional practice educators qualification
* Evidence of the ability to work with data to produce reports and business cases
