12 Month Fixed Term Contract
Responsibilities
* Demonstrate expert knowledge of discharge options and services available.
* Provide support to the patient, family/carer and care workers in communicating the discharge plan, estimated date of discharge and managing expectations and concerns.
* Aim for a safe and sustainable discharge by highlighting and managing any potential risks to reduce the risk of readmission.
* Contribute at multidisciplinary team meetings to ensure the patient’s wishes are heard and needs are assessed and managed to achieve a timely discharge.
* Promote advanced care planning where appropriate to avoid unnecessary hospital admissions.
* Identify social care needs of older people and be aware of, or able to access, adult social care processes that facilitate discharge from hospital.
* Act as a link between wards and community teams/services for discharge planning.
* Complete specialist discharge planning and liaise with the discharge planning nurse to complete specialist nursing assessments.
* Work closely with healthcare professionals, clinicians, social work teams, family members, personal care services and other multidisciplinary team members to deliver a high‑quality, patient‑focused service.
Qualifications
Experience in discharge planning and the ability to work across multidisciplinary teams are essential. Demonstrated specialist discharge planning skills and knowledge of relevant services are required.
