We have an exciting opportunity within the team for two registered professionals to join the Learning Disability Crisis Response and Intensive Support Team as a Band 6 practitioner—one post at 37.5 hours a week and one at 22.5 hours a week, or two posts at 30 hours a week. The team provides a service Monday to Sunday, 8 am to 9 pm, including bank holidays, based at the Agnes Unit. The successful candidates will have the opportunity to discuss flexible working patterns within these hours.
You will hold your own caseload and work within a specialist team comprising nurses, healthcare support workers, Occupational Therapists, Speech and Language Therapists, Psychiatry, and Psychology, with input from allied health professionals—part of the wider Learning Disability Service.
The Learning Disability Crisis Response and Intensive Support Team offer support, clinical assessments, interventions, and risk management for individuals with learning disabilities at risk of inpatient admission or losing community placements. Support is based on Positive Behavioural Support and intensive mental health support for individuals with complex needs living in Leicester, Leicestershire, and Rutland.
This role is ideal for a registered professional with experience working with people with learning disabilities, particularly those with high-risk needs and community experience. It offers opportunities to develop clinical skills within a passionate team dedicated to supporting individuals to live successfully in the community.
The successful candidates will receive comprehensive induction and access to bespoke training related to assessment and risk management of patients with learning disabilities and complex behavioral needs.
You will need a full driving license and access to a vehicle insured for work purposes.

Job Description Summary (Main Duties and Responsibilities)
* Hold a caseload of patients with complex needs, conducting specialist risk assessments and interventions.
* Triaging new referrals from various sources, including patients, carers, and professionals.
* Provide clinical assessments, support, planning, and oversight for patients at risk of admission or placement breakdown.
* Act as the lead professional, completing assessments and maintaining records.
* Collaborate with inpatient teams, commissioners, and other case managers.
* Support Community Learning Disability Teams with high-risk cases through wrap-around support and advice.
* Offer training and advice to carers, providers, and health and social care staff regarding individual needs and Positive Behavioural Support Plans.
* Provide specialist advice to enable individuals to remain in the community and prevent inpatient admissions where possible.
* Deliver direct interventions when required.
* Monitor and support using pathways such as the Mental Health Pathway and Positive Behavioural Support Pathway, transitioning cases to LD Community teams when risks are manageable.
