Home Care Practitioner (Workington)

Part‑time – 36 hours per week (2 posts available)
Band 3 Home Care Practitioner role within Workington Integrated Care Community, working alongside the Community Team with a special interest in Palliative Care and Rehabilitation.
A motivated, caring individual who is enthusiastic about rehabilitation, provides person‑centred care and enjoys working in a team is preferred.
Required to work over 7 days; shift pattern rota covering 8 a.m.‑10 p.m.
Must be able to travel independently across a wide rural area.
Job responsibilities
Work as an active member of the multidisciplinary team (MDT) providing personal care activities and delegated clinical care duties, including clinical observations and specified clinical skills as directed by registered professionals, within clearly defined parameters. The goal is to maximise independence, confidence and quality of life for patients in their own homes.
Help facilitate early discharge from hospital or prevent unnecessary admission through close working relationships with local health and social care teams.
Qualifications
* Numeracy and Literacy Level 2
* National Care Certificate or commitment to complete within 12 weeks of commencing employment
* Knowledge of data protection legislation and health and safety at work legislation
* Level 3 vocational qualification or equivalent experience in a range of patient and clinical care duties, including theoretical knowledge and clinical observations
* Undertaken relevant additional training for Clinical Support Workers
* Excellent interpersonal, communication and active listening skills
* Ability to adopt appropriate styles and methods of communication
* Time‑keeping, planning and prioritisation skills
* Basic IT skills to use electronic patient records and Trust systems
* Analytical and judgement skills to make decisions based on patient interactions, observations and metrics
* Ability to demonstrate the importance of holistic person‑centred care and related procedures
* Ability to use initiative appropriately
* Ability to demonstrate compassionate values and behaviours needed for dignified care
* Desire to encourage and support patients to remain as independent as possible
* Ability to deal with a wide range of people at all levels, including clients, carers, family, health and social care colleagues and other professionals

Knowledge and Experience
* Experience of working in health or social care
* Experience of undertaking and recording clinical observations and escalating changes or findings
* Working knowledge of legislation underpinning care provision
* Understanding of the importance of robust, accurate, clear, timely documentation
* Insight into behaviours and skills of a carer and the need for effective teamwork
* Knowledge of Safeguarding
* Understanding work‑related boundaries
* Awareness of GDPR and confidentiality requirements
* Familiarity with current NHS and social care policies
* Nursing policies and procedures
* Evidence of understanding of Trust business
* Experience of working in the Trust previously
