What the Job Involves As a Social Worker, you will conduct face-to-face assessments, including Mental Capacity Assessments, at the point of discharge. Your role involves providing critical information, advice, signposting, and arranging home care or residential care. You will contribute to effective discharge planning and gain experience in multi-disciplinary collaboration. Adhering to the Care Act 2014, you will ensure the safeguarding of adults and carers at risk, promoting well-being and independence. The role includes both providing social work assessments and promoting continuous development and staff well-being.
