What the Job Involves As part of the Hospital Discharge Team at Rhondda Cynon Taf County Borough Council, you will be responsible for supporting individuals and families through the discharge process from four main hospital sites. The role requires you to work full-time, 37 hours per week, within a hybrid working model, including home working and attendance at hospitals and council office bases. Key responsibilities include undertaking risk assessments, completing strengths-based assessments and care plans, supporting outcome-focused discharge approaches, and working collaboratively with health professionals. Skills Recognized Social Work qualification Registered with Social Care Wales Knowledge of Social Services and Well-being (Wales) Act 2014 Understanding of Discharge to Recover and Assess (D2RA) Familiarity with Mental Capacity Act 2005 Experience with Deprivation of Liberty Safeguards (DoLS) (2007) Understanding of Continuing Healthcare Framework Strong assessment and care planning skills Ability to work collaboratively with various stakeholders
