Supporting young people from age 14 as they move from children's to adult services, the Transitions Team ensures those eligible under the Care Act (2014) receive effective support at the right time where adult social care can add value to their lives. The team works closely with Children's Social Care, SEND services and health partners to secure the best outcomes for young people entering adulthood.
Responsibilities
* Provide day‑to‑day professional leadership to both teams, supporting Team Managers, Senior Social Workers and practitioners.
* Monitor performance, ensuring KPIs are met and addressing any issues promptly.
* Lead on quality assurance of assessments, support plans, Mental Capacity Assessments and other key documents – offering constructive feedback to maintain high standards.
* Identify skills gaps and support the development of practitioners and emerging leaders.
* Contribute to service‑wide projects and continuous improvement initiatives.
* Handle complaints and embed learning across the teams.
* Work with commissioners and children’s services to shape strategic developments.
* Oversee safeguarding enquiries, ensuring timely and appropriate outcomes.
* Build effective partnerships by contributing to strategic groups.
* Support sound financial management of community care and staffing budgets.
* As a senior leader, provide professional oversight, drive high‑quality practice and strengthen relationships with local partners.
* Work alongside two Team Managers and the Strategic Service and Delivery Lead, play a key role in service development, continuous improvement and achieving the best possible outcomes for Staffordshire residents.
Qualifications
* Hold a professional Social Work qualification and be registered with Social Work England.
* Have substantial experience since qualifying as a Social Worker in a leadership/management role as well as a practitioner.
* Have an understanding of the relevant legislation that frames statutory Social Work practice.
Appointment Details
This is a fixed‑term role to cover maternity leave, starting on 1st May 2026 until 31st May 2027. The post is full time for 37 hours per week. Core business hours are 9 am to 5 pm, Monday to Friday.
Working Arrangement
Hybrid working is available, offering a mix of office and home‑based work. The services support people living across Staffordshire and the teams work from local bases across the county, mainly in Stafford and Burton‑on‑Trent. Your office base is negotiable, however you will be required to be present in one of the two main bases during the working week.
