Overview
An exciting opportunity has arisen for a Co-existing Mental Health / Substance Misuse worker within the Clayfields Health Team. The postholder will take a lead role in providing the successful and effective delivery of Substance Misuse services and Mental Health in Clayfields Local Authority Secure Children's Home. To work as a Co-existing Mental Health/ Substance Misuse nurse within a multi-disciplinary team in a Secure Children's home setting. Providing care to young people with complex substance misuse and mental health problems, from the point of entry into Clayfields House to re-entry into the community to ensure that offending is reduced and the health and social functioning of young people is improved.
To provide high quality patient care through assessing, planning, implementing and evaluating care for young people with substance misuse and mental health co-morbid problems working towards both harm reduction and health promotion / recovery agendas.
Responsibilities
* To care co-ordinate a complex case load including those with significant mental health needs, with responsibility for assessing, planning, implementing and evaluating patient care.
* To assist in the development of systems, pathways and policies which promote delivery of services to co-existing mental health/ substance misuse diagnosis Children and Young People.
* To undertake comprehensive assessment of patients\' needs following referral to the service particularly those who have identifiable mental health issues.
* To plan programmes of clinical and psycho-social interventions using evidence-based practice. To decide upon and recommend the best course of action and plan care for co-existing Mental Health and Substance Misuse patients.
* To provide information and advice about drugs, alcohol, smoking cessation and related issues, to include blood bourne viruses, safer sex and mental health.
* To provide high quality patient care through assessing, planning, implementing and evaluating care for young people with substance misuse and mental health co-morbid problems working towards both harm reduction and health promotion / recovery agendas.
