Successful applicants will be required to provide an immunisation and vaccination report, from either their current UK occupational health provider or their UK GP. We welcome applications from Occupational Therapists currently practising, returning to practice or those that are newly qualified and have an interest in Discharge. Working alongside an interdisciplinary, knowledgeable and supportive team the successful candidate will have the opportunity to develop complex assessment and decision-making skills in discharge. The post holder will provide assessments, coordinate and refer patients for support along their ongoing journey of recovery and / or reablement to maintain, and restore functional abilities whilst considering the individuals’ abilities, health requirements, and environment. The post holder will demonstrate excellent communication, interpersonal and team-working skills. The post holder will be expected to manage a caseload, plan, and prioritise assessments, whilst demonstrating clear clinical reasoning and evidence-based practice, with support from their team and senior colleagues. The post holder will actively engage to promote the role of Occupational Therapy and the Integrated Discharge Team Service. The post holder will provide clinical support to less-experienced staff, as delegated by seniors.
