Care Co-ordinator
The Care Co‑ordinator is a pivotal role within primary care, serving as the interface between service users, carers, primary, secondary, community and social care, mental health, out‑of‑hours and voluntary organisations. The role is part of the Belper Integrated Community team and works alongside the Belper GP practices.
The position is an administrative, non‑clinical role suitable for candidates with health or primary care experience. We seek excellent communicators with strong time‑management skills, enthusiasm and motivation to improve people's lives.
Main duties of the job
* Maintain IT‑based information systems and generate relevant performance data.
* Act as liaison between service users, carers, and the various health and social care services.
* Support efforts to tackle inequalities in health and social care, particularly for individuals with long‑term conditions.
* Promote independence and partnership‑working throughout the role.
Experience and knowledge required
* Relevant experience working in healthcare, NHS or primary care.
* Clear understanding of confidentiality and handling sensitive information.
* Experience working in a multi‑disciplinary setting, including influence and negotiation.
* Familiarity with database use and medical terminology.
* Experience providing care for vulnerable adults and the elderly.
* Knowledge of health and social care processes and training in continuing care criteria.
* Ability to work in a busy, demanding environment and deliver tasks timely.
Education, qualifications and training
* NVQ Level 3 Business Administration, EDCL, HNC or equivalent.
* Ongoing internal and external training to stay current with industry developments.
* Long‑term conditions training and welfare rights basic training.
