Responsibilities
* You MUST have past experience of working as an ANP in a GP surgery in the UK.
* Attend to patients who wish to be seen by a health care professional; diagnose and treat patients presenting with minor illness; advise on continuing medical and nursing needs.
* Prioritise and triage the needs of patients; make referrals for investigations as appropriate.
* Work with practice protocols and guidelines; provide nursing treatments to patients independently and in collaboration with nurse colleagues and general practitioners.
* Provide access to a Nurse Practitioner in surgery and by phone for all presentations in primary care.
* Manage admission and discharge to your caseload through comprehensive assessment (physical, mental, emotional and social health) and the use of investigations to establish a diagnosis.
* Prescribe evidence-based care including medication, where qualified to do so, and according to formulary and competencies.
* Develop ongoing health plans with patients and families, with emphasis on prevention and self-care; arrange follow-up and evaluation of care.
* Direct referrals and liaison with consultants and other professional agencies; participate in referrals within the practice team.
* Support patients to adopt health promotion strategies, and apply self-care principles; support women presenting for family planning or sexual health consultation; assess and refer patients with mental health needs.
* Maintain accurate patient records on the computer system.
* Undertake pathological specimen collection and investigatory procedures as requested by GPs; check and process results as instructed.
* Disseminate learning to the team, manage risk, participate in Chronic Disease Management and achieve QOF targets.
* Ensure effective communication within the team; participate in clinical governance and audits; support and facilitate team working and the development of protocols and clinical guidelines.
Qualifications
* Essential
o Registered General Nurse (currently registered with the Nursing & Midwifery Council)
o Recognised ANP qualification at minimum degree level or equivalent
* Desirable
o Independent Nurse Prescriber
Experience
* Evidence of continuing professional development in all aspects of the ANP role
* Experience in management of long-term conditions
* Experience working autonomously and as part of a team
* Ability to evaluate safety and effectiveness of clinical practice
* Experience with minor illness, clinical management and patient referrals
