Job Summary
An exciting opportunity has arisen for a full time First Contact Mental Health Practitioner with a specialist interest in working with young people, to work within primary care within the South Hambleton and Ryedale Primary Care Network (SHaR PCN).
This role is fundamental to the development of inclusive and integrated mental health services. The post holder will work in GP practices across the PCN, providing a first contact specialist mental health service and collaborating with multiagency teams to support local population wellbeing. Travel across the locality is anticipated.

Job Responsibilities
* Provide expertise in networking and navigation to promote access to specialist mental health support services.
* Undertake initial assessments with signposting to appropriate services; provide time-limited psycho-social interventions in GP surgeries, patient homes and other primary care settings.
* Offer clinical advice to GPs, act as a specialist advisor, and manage pathways in collaboration with GP teams and other practice-based mental health workers.
* Develop partnerships with key stakeholders across the PCN and wider mental health care system; advise on interventions and ongoing patient care management.
* Foster partnerships between Primary Care Teams and Mental Health Services; facilitate joint work with PCN, Secondary Care, and Voluntary Sector to expand services available to communities.
* Provide clinical advice and support to primary, secondary, voluntary and statutory services as required.
* Collate information about local opportunities and resources that benefit referred patients (e.g., voluntary/ community organisations, educational and recreational opportunities).
* Signpost to appropriate services and provide very short-term interventions for mild to moderate mental health difficulties (average duration ~6 weeks); work with PCN staff including care coordination teams and social prescribing link workers.
* Collaborate with the PCN, GPs and Social Prescribers/other practice-based mental health workers on mental health needs of the adult population; ensure smooth transitions between primary and secondary care pathways.
* Travel independently across the locality and have access to a suitable vehicle for business purposes; adjustments considered under the Equality Act 2010 as needed.
Person Specification
Qualifications
Essential
* Current professional registration with an appropriate healthcare or social work body.
* Evidence of Continuing Professional Development.
* Certificate in mentorship, teaching & assessing in clinical practice as appropriate to own profession.
* Clinical Supervisor and other requirements as applicable to professional registration.
* Key skills in literacy, numeracy and IT at Level 2 or equivalent.
Desirable
* NMP (or willing to obtain) if appropriate to professional registration.
* Leadership or management qualification.
* Post-graduate qualification in a relevant area.
Experience
Essential
* Significant experience working with people with mental ill health in a community setting.
* Experience working collaboratively with service users and their families/carers.
* Providing clinical supervision; mentoring students on practice placement.
* Experience working in a multi-disciplinary team; involvement in quality improvement activities.
Desirable
* Experience working in primary care; experience in a multiagency setting; leadership or management experience.
Knowledge
Essential
* Post-graduate level knowledge of evidence-based practice in the designated field.
* Knowledge of the Stepped Care model; safeguarding, Mental Health Act, Mental Capacity Act, Care Programme Approach, clinical risk assessment and management, clinical governance, and related policies.
* Knowledge of delivering psycho-social interventions and contemporary health and social care policy.
Skills
Essential
* Ability to communicate complex information effectively to patients, carers and the multidisciplinary team.
* Work effectively as part of a multidisciplinary team; undertake lead professional responsibilities.
