Contract Type: Fixed Term for 1 year. Working Hours: 33.25 hours per week. Worker Type: Night Care Worker. Location: The Oaks, St Anns, Nottingham, NG3 1GZ. Salary: Starting £26,824 for Level 1 rising to £27,694 for Level 3 per annum (pro rata for part-time).

About The Role: The Oaks (St. Ann’s) is a modern residential unit for elderly citizens with a range of needs, including dementia. Citizens may be short term or longer term residents depending on their identified needs and circumstances.
Care Workers at Level 2 implement Care Plans and report any concerns or reasons care plans cannot be followed, monitor care and welfare of citizens, and record accurate information in running records, communication books, care plans monitoring charts (e.g., weight monitoring, falls) and other written/electronic communications as required. They work with other professionals, citizens and their families to ensure a shared approach to care and continuity of care and treatment. They provide safe and appropriate personalised care that maintains dignity, maximises independence and gives people choice and control over how their care is provided, including writing and maintaining individual person-centred care plans and the administration of medication. They may work in any area of Adult Social Care, in the home, the community or within a Council establishment and should be prepared to support Citizens with different and varying needs.
Responsibilities
* Assist and support citizens to exercise choice and control in accordance with their needs, preferences and aspirations.
* Encourage, motivate and support citizens to maintain or regain maximum independence in daily living and meaningful activities.
* Develop and coordinate leisure and social activities that promote self-esteem, confidence and a purposeful day, including one-to-one or group activities and maintaining contact with family, friends and the community.
* Write and maintain up-to-date individual person-centred care plans and assist with administration of medication as required.
* Work collaboratively with other professionals to ensure continuity of care and treatment.
* Adhere to safeguarding and duty of care requirements and maintain accurate records.

* Ability to motivate citizens to maintain or regain independence in daily living and activities.
* Ability to develop and coordinate leisure and social activities that promote self-esteem and a purposeful day.
* Ability to write and maintain up-to-date, individual person-centred care plans.
* DBS enhanced check for a regulated activity is required for this post.
