An exciting opportunity has arisen for a Social Prescriber to join our well-established team at ARC PCN.
About our PCN
Our PCN comprises 6 Buckinghamshire GP practices, collaboratively working together for their population of 85,000 patients:
* The Simpson Centre and Penn Surgery
* Millbarn Medical Centre
* Bourne End and Wooburn Green Medical Centre
* Highfield Surgery
* Cherrymead Surgery
* Marlow Medical Group
What experience will you need?
* Understanding of social prescribing or a person-centred approach.
* Ability to be a good listener, have time for people, and support local communities.
* Experience working with individuals facing complex social and emotional challenges (preferable).
* Great interpersonal skills and a proactive approach to supporting people, community groups, and organizations.
* Live locally in the community.
* IT literate, as multiple clinical systems are used.
Main duties of the job
Social prescribing involves giving people time to focus on what matters to them and taking a holistic approach to health and wellbeing. It connects individuals to community groups and statutory services for practical and emotional support.
This role empowers people to take control of their health through GP referral to Community Link Workers (CLWs), who provide time, focus on individual needs, and connect them to community and statutory services. CLWs work collaboratively with all local partners.
Social prescribing helps strengthen community and personal resilience, reduces health inequalities by addressing wider determinants of health (like debt, housing, physical inactivity), and is especially beneficial for those with long-term conditions, mental health support needs, loneliness, or complex social needs.
You'll work with diverse individuals of all ages and backgrounds, with each day offering new interactions and opportunities.

Job responsibilities
* Provide personalized, holistic support to individuals, families, and carers to promote independence and improve health outcomes.
* Promote social prescribing's role in improving health, reducing social isolation, and supporting independent living.
* Build relationships with GP practice staff within the PCN, attend meetings, and provide feedback on social prescribing.
* Partner with local agencies to raise awareness of social prescribing, reduce pressure on statutory services, and enable holistic care.
* Provide referrers with regular updates on social prescribing, including benefits, referral processes, and outcomes.
* Gather feedback on service quality and impact from referrers.
* Collaborate with the Prevention Matters service to maximize social prescribing availability and prevent resource duplication.
* Encourage feedback and stories from patients, families, and carers about the impact of social prescribing.
