The Patient Flow Coordinator plays a pivotal role in ensuring the safe, timely, and efficient movement of patients through our community hospitals. This is an exciting new role which will require you to communicate effectively with patients, families and our multidisciplinary team in order to help patients get home as soon as they are ready. You will work with the teams on both City Community Hospital ward in Oxford, and Bicester Community Hospital. Your work will directly contribute to reducing length of stay, preventing readmissions, and improving patient outcomes. Main duties of the job Key responsibilities of this role include: * planning and coordinating hospital discharges * effective communication with patients and families to involve them in preparing for discharge from hospital * working closely with colleagues in other organisations such as adult social care and voluntary sector organisations to arrange appropriate support for patients when they leave hospital * maintaining clear and accurate documentation * coordinating and contributing to daily board rounds and weekly MDT meetings No two days are the same for the patient flow team. Essential criteria * Ability to travel independently between sites within the trust * Educated to NVQ Level 3, BTEC, or equivalent qualification/experience in health or social care * Understanding of effective discharge planning and transfer of care processes * Experience in a health or social care setting, ideally within a hospital or community environment * Experience of working as part of a multidisciplinary team Desirable criteria * Car owner and driver * Evidence of ongoing professional development * Familiarity with community care services, resources, and eligibility criteria * Experience of discharge planning or coordination Job related skills Essential criteria * Ability to communicate clearly and empathetically with patients, relatives, and professionals * Strong organisational and prioritisation skills, able to manage multiple cases simultaneously * Shows effective problem-solving and decision-making skills, with the ability to escalate appropriately * Confident IT user, with ability to record, track, and report information using electronic systems
