Main duties of the job
Key duties:
 * Interplay between Same Day Emergency Care / Hospital at Home / Single Point of Access and Urgent Community Response (UCR) covering 7 days per week.
 * Work collaboratively with all reablement providers to support monitoring for those people who need a period of recovery prior to rehabilitation and reablement at home.
 * Provide strong clinical support and help shape the future of hospital care in the community.
 * Transform Hospital care in the community and develop an integrated model of care.
