About Us

The Integrated Discharge Service (IDS) works closely with Health Professionals and Age Connects to provide discharge planning support to all citizens 65 years and over resident in the Vale of Glamorgan whilst they are in hospital.

The Team is based at University Hospital Llandough (UHL) and consists of Social Workers and Accommodation Discharge Co‑ordinators who work in partnership with Health Professionals and Discharge Support Officers (Age Connects) to assist in planning and arranging the safe discharge of adults from the Princess of Wales Hospital, University Hospital of Wales (St David’s Hospital Cardiff), University Hospital Llandough and Barry Hospital. The Social Work team provides a liaison between the hospital wards, citizens and their families to develop a Care and Support Plan that enables identified needs to be met and focuses on wellbeing outcomes being promoted in order to maintain safety, independence and quality of life when discharged from hospital. Social workers work as part of the multidisciplinary team (MDT) attending safe discharge planning meetings, best interest meetings and Continuing Health Care Meetings to facilitate safe timely discharge.


The Role

Pay Details: Grade 8/9, SCP 26 - 35 £37,280 - £46,142 Pro Rata £19.32 - £23.92 Per Hour

Hours of Work: 37

Working Pattern: Monday to Friday

Main Place of Work: Llandough Hospital

Description: To assess adults to determine eligibility for the provision of services, promote user empowerment and independence, and offer appropriate information, advice and sign‑posting to alternative services to meet identified needs.


About You

* Experience of working with adults who are vulnerable through illness, disability, mental illness, substance mis‑use or age‑related frailty.
* Experience of working with adults living with mental health problems, and within mental health services.
* Assessment of adult service users and carers.
* Understanding of illness, disability, mental illness, substance mis‑use and age‑related frailty and its effects on individuals, carers and families.
* Knowledge of relevant legislation such as the Social Services and Well‑being (Wales) Act 2014, the Mental Health Measure (Wales) 2010, the Mental Capacity Act 2005 and the Mental Health Act 1983.
* Knowledge of safeguarding issues and procedures in relation to adults.
* A good standard of general education.
* A recognised Degree/Diploma or equivalent qualification in Social Work (e.g. Degree, DipSW, CQSW).
* Registration with Social Care Wales.
* DBS Check Required: Enhanced & Barred Both.
