The service operates a 24/7 Home Treatment Team (HTT). You will work autonomously in the community, complete full mental health assessments, manage risk, and be an integral part of the multidisciplinary team. You will lead on coordinating shifts, delegate to team members, and liaise with key stakeholders. You will respond urgently to requests for assessment and treatment during a mental health crisis and act as gatekeeper for admissions to inpatient services, providing an alternative to hospital admission through Home Treatment where appropriate.
You will be based in the Home Treatment Team (CHTT) and be the first point of contact for referrals, while working as part of a dynamic multidisciplinary team to support service users in acute mental health crisis in their homes and support families and carers.
Responsibilities
* Provide a comprehensive assessment of a service user’s mental state and social needs to inform a collaborative care plan for community management.
* Perform comprehensive risk assessment, formulation, and management as part of the service user’s journey.
* Act as keyworker for a small caseload with support from the multidisciplinary team.
* Cover the duty/shift coordination role regularly.
* Liaise with other agencies involved in a service user’s care.
* Support and supervise junior staff.
* Lead on discharge or transfer of care to appropriate agencies.
* Adhere to professional confidentiality and the register body’s code of conduct.
* Discharge responsibilities under the Mental Health Act and the Mental Capacity Act; follow the NMC Code of Conduct.
* Be accountable for own practice and manage medicines in line with NMC and Trust policies.
* Maintain records and communicate effectively with inpatient services, social services, IRHs, and other statutory/non-statutory organisations to ensure continuity of care.
* Maintain knowledge of legal requirements including the Mental Health Act, Mental Capacity Act, Care Act, Deprivation of Liberty Safeguards, Safeguarding Vulnerable Adults, and Community Treatment Orders.
* Engage in relapse prevention, support self-management, and contribute to crisis planning with clients.
Qualifications and Experience
* Essential: RMN (current NMC registration); evidence of continuing professional and personal development; full and current driver's licence and car owner (or willingness to participate in the Trust car lease scheme).
* Desirable: Mentorship/ENB 998 or equivalent; RGN; Diploma in cognitive behavioral therapy/psychodynamic counselling.
* Experience: demonstrable experience of working with people with serious ongoing mental health problems in a community or outpatient setting post qualification; experience in multidisciplinary teams; experience of assessing and managing crisis; experience as a clinical supervisor.
