JOB TITLE: Registered Nurse (RN) / School Nurse
DEPARTMENT: Nursing
REPORTS TO: Nurse Director
PURPOSE: Registered Nurses regardless of specialty or work setting, treat and educate individuals, families and the public about various medical conditions. The RN is a core member of the interdisciplinary care team for the children and adults we support with Complex Medical Conditions. The RN is guided by the Nurse Practice Act and the Code of Conduct for Nurses. The RN staff nurse will utilize the nursing process, critical thinking, and problem solving skills to provide holistic care to the individuals we support, their family members and communities and population across the life span.
Functions And Responsibilities
Safety & Well-Being of Individuals
Outcome: All Critical Life Safety Programs and policies are trained, implemented, documented, followed up on and evaluated in alignment with The Center for Discovery philosophy.
* Provide initial and ongoing assessment and interpretation of individuals within the framework of holistic professional nursing practice and in accordance with the principles of individual and family centered care
* Assess and interpret information and data regarding individual and families’ culture, value system, lifestyle and the interest and ability to participate in their care, and any other factors impacting successful treatment outcomes
* Assess and interpret the individual’s ability (physical, social and psychological) to manage their health in order to transition their care upon discharge and/or adult placement. In collaboration with the individual, family members and interdisciplinary team, identify any needs based on analysis of data
* Identify and communicate individual and family needs to the interdisciplinary health care team, and other providers as needed
* Provide ongoing monitoring of individual or family response to intervention, identify deviations from expected outcomes and asses change in individual status
* Assess situation(s) involving safety and health related issues for the purpose of identifying problems, referring for proper treatment, and complying with legal requirements.
* Reports incidents for the purpose of maintaining student’s personal safety and adhering to the policies of the Center for Discovery.
* Responds to emergency situation for the purpose of addressing immediate safety concerns
Individual Life Planning
Outcome: Develop and implement a comprehensive person specific nursing plan of care in collaboration with the interdisciplinary team and aligning with The Center for Discovery Philosophy.
* Develop, coordinate and update an individualized plan of care, based on evidence in collaboration with the individual and their family, the interdisciplinary team and medical home providers promoting continuity of care across the continuum
* Prioritize nursing interventions to achieve individual outcomes and goals
* Develop and coordinate a comprehensive individual/family education plan to meet the individual’s and family’s needs. Inclusive of health promotion, self-management, identification and coordination of resources to promote the continuity of care
Intervention
Outcome: All interventions are implemented, documented, and evaluated in alignment with The Center for Discovery Philosophy.
* Establish and maintain a therapeutic relationship with individuals and families
* Identify, initiate and coordinate therapeutic interventions consistent with the scope of practice and level of competence
* Implement prescribed interventions and perform independent nursing interventions
* Consult with the interdisciplinary team and medical home providers to discuss changes related to individual status and modifies interventions
* Interpret medical information inclusive of diagnostic testing and treatment plans to assess individual’s response
* Provide information and instruction to assist individual to understand health care practices and processes.
* Utilize evidence-based educational materials to supports individuals and families in understanding their health care needs, promotion and maintenance to enhance their individual well-being
* Collaborates with parents, students, and education staff for the purpose of promoting health and wellbeing.
* Provide training on a variety of health related subjects for the purpose of acting as a resource to teachers and other school personnel.
Evaluation
Outcome: Ensure that individuals plan of nursing care is appropriate based on relevant data and evidenced based best practices.
* Utilize data obtained from individual, family, and other health care providers as well as internal data collected to evaluate the plan of care including individual responses to interventions and achievement of expected outcomes
* Support direct care staff members and educate appropriately any planned changes in care with the individual
* Use data and evidence-based best practices to evaluate nursing activities and interventions
* Document complete, accurate, and relevant data for every individual assigned to their care for the shift as well as assigned primary individual caseload
* Prepare reports and correspondence as required for the purpose of documenting activities, providing written reference and/or conveying information
* Participate in a variety of meetings, conference days, IEP meetings for the purpose of sharing information and/or improving skills and knowledge.
* Assist other personnel as required for the purpose of support and completion of work activities.

Outcome: Reflect The Center for Discovery’s and New York State’s Core Competencies Standard for the day to day practice of supporting people with Developmental Disabilities and Complex Medical Conditions in conjunction with the Nurse Practice Act.

* Participate in the development and revision of standards of care, policies and procedures as deemed necessary
* Promote a safe work environment by identifying and resolving potential risk issues
* Demonstrate and awareness of organizational needs by participating and establishment of priorities, resources and environment
* Function as a change agent by thinking respectively, questioning assumptions, assessing alternatives and supporting change
* Identify and participates in problem or conflict resolution utilizing appropriate strategies, process and procedures
* Guide clinical learning experiences as a preceptor for students and staff as assigned and supports the orientation of new staff members by sharing nursing knowledge and contributing to a supportive learning environment
* Participate and utilize research based strategies to improve outcomes of the individuals served
* Accept responsibility for continuing competencies through ongoing professional development including participation in education programs, research and continuous quality programs
* Accept administrative responsibilities as assigned and accepts responsibility for resource management within all areas of responsibility
* Organize and administer the school health office for the purpose of ensuring that the area functions in an effective manner.
Qualifications
* Graduate of an Associates of Science or higher degree from and accredited institution offering a curriculum in nursing
* Two years of work experience in nursing preferred with expertise based on prior experience with individuals with intellectual or physical disabilities
* Demonstrate skill and expertise in utilization of the nursing process, individual-centered medical home and interdisciplinary team approach
* Maintain required licensing and mandated training by the NYS Education Department

Licensure/Certification
* A Registered Professional Nurse currently licensed in the State of New York
* Current Basic Life Support Certification
