Qualified Social Worker, Discharge to Assess Team, Blackburn CouncilPay rate £30 per hourContract roleVitalis are recruiting for experienced Social Worker to work in a Discharge to Assess Team in Blackburn Council.We are looking for a skilled and motivated Social Worker to join our Hospital Discharge and Discharge to Assess (D2A) service. This is a dynamic, fast-paced role focused on supporting timely, safe, and person-centred discharges from hospital into community settings via Discharge to Assess pathways. ASAP Start Working as part of a multi-disciplinary team, you will play a key role in ensuring individuals leave hospital with the right support in place, enabling them to recover and regain independence in the most appropriate environment.This is a hybrid role, offering a balance of office based, on-site hospital work, community visits, and remote working.Don’t forget, if this role isn’t the perfect fit, we’re confident we can find one that is.
