Overview
Community Nurse – Learning Disabilities – Band 6 at Birmingham Community Healthcare NHS Foundation Trust (BCHC).
This advert is for posts available across the city at three bases: Kings Norton, Small Heath and Erdington. Preference of base will be discussed at the offer stage.
Responsibilities
* Work within the framework of a multi-disciplinary team to provide support and guidance to children and adults with learning disabilities and their carers.
* Work within the framework of health facilitation (level one and two).
* Coordinate a clinical service under the supervision of Senior Community Nurses.

Requirements & Qualifications
* Experienced and flexible individual with excellent communication skills and a passion for multidisciplinary teamwork.
* Demonstrated ability to work within a multi-disciplinary team and to coordinate clinical services under supervision.
