Overview
Band 5 Community Nurse - Extended Primary Care Teams, Newham. This is an opportunity for experienced and newly qualified Registered Nurses (Adult) to join the Extended Primary Care Teams in Newham, East London as Band 5 Community Nurse.
The service operates in East Ham Care Centre and is available 365/366 days a year, including weekends and Bank holidays from 8am to 8pm.
What we are looking for
* Experienced qualified nurses (Registered Nurses Adult) who want to develop their skills and knowledge in community health practice.
* Newly qualified nurses seeking their first post as a qualified nurse.
* Nurses who offer a high level of person-centered nursing care.
* Nurses with excellent verbal and non-verbal communication skills.
* Nurses who can work as part of a multidisciplinary team to enhance care quality.
* Nurses who can work shifts over 7 days a week (including Bank Holidays).
Responsibilities
* Provide nursing care to patients in their own homes, with autonomy.
* Plan, implement, and evaluate individualised nursing care in a range of settings.
* Provide health education to enable patients to self-manage and regain independence after deterioration or illness.
* Support the senior nursing team in maintaining efficient and effective delivery of services.
* Work closely with multidisciplinary teams including GPs, social workers, therapists, and specialists.
* Coordinate care to ensure continuity and prevent hospital admissions.
* Communicate effectively with patients, families, and healthcare professionals.
Qualifications and Experience
Essential
* Registered General Nurse (Adult).
* Demonstrate evidence-based practice and up-to-date clinical skills.
Desirable
* Evidence of post-basic professional development.
* Relevant teaching/mentorship qualification or commitment to undertake.
Experience
* Evidence of and commitment to multi-professional working.
Knowledge
* Literate in IT/Computer Skills.
Other
* Ability to travel across sites and Trust boundaries to meet patient needs and attend meetings.
* Current driving license (Desirable).

This post is subject to Disclosure and Barring Service (DBS) checks; applicants must be eligible to work in the UK.
Applicants must have current UK professional registration with the NMC and may be considered if in the final year of training and meeting other essential criteria.
