The successful candidate will play a pivotal role inthe co-ordination and delivery of acute and unplanned care to our community.

Advanced Paramedic Practitioner / Emergency Care Practitioner

Providing urgent same-day care to patients in their own homes

To assess and manage patients independently, with the support of the GPson the Acute Day Team

To contribute to the development of the Acute Day Service at the Practice

Main duties of the job

You will work autonomously and within the competency level of anAdvanced Paramedic Practitioner / Emergency Care Practitioner. You will bebased within the Practice at Pembroke House working alongside and supported bya team of GPs and AHPs as part of the Acute Day Team, providing urgent same-daycare.

The role will involve the assessment, diagnosis, treatment, and / or onwardreferral of patients presenting with undifferentiated and undiagnosedconditions. Please note the role will predominately involve assessing andtreating patients in their own homes. In time, there may be the opportunityto progress to providing clinics within the Practice, depending on theindividuals skills and experience and our operational requirements.

Pembroke Medical Group is part of the Baywide Primary Care Network,which includes Compass House Medical Centre in Brixham and Chilcote Surgery inTorquay. Together we serve a population of 50,000 patients. The expectation isthat the successful candidate will collaborate with GPs, Nurse Practitioners,Mental Health Practitioners, Pharmacists, Practice Nurses, as well as ourcolleagues in Community Nursing, Physiotherapy, Occupational Therapy, SocialCare and at SWAST.


Job responsibilities

Clinical

* To provide assessment, diagnosis, and treatment of patients at first point of contact, predominately in their own homes.
* To accurately triage and prioritise patients, including in emergency situations.
* To apply the necessary skills and techniques to elicit a clear history of events / episode of illness, including the relevant background medical, medications and social histories.
* To evaluate clinical information from history taking and examination to arrive at differential diagnoses, and to initiate an appropriate treatment and /or referral.
* To undertake and interpret diagnostics and take appropriate action to manage patient care. To distinguishing between abnormal and normal findings and to relate these findings to a working diagnosis. To use broad clinical guidelines (NICE etc.), relating them to individual cases and feeding back to colleagues on how these should be interpreted.
* To appropriately signpost patients to other services for their identified healthcare needs.
* To refer patients to appropriate care settings.
* To treat patients as appropriate within scope of practice, and to ask for support when unsure.
* To maintain accurate and systematic documentation of all patient interactions and clinical care provided, including adverse events.
* To advise and support patients on healthy lifestyles, health promotion and disease prevention.
* To provide clinical advice to other members of the team where appropriate.

Communication

* To liaise and interact with multidisciplinary teams across organisational boundaries, including primary care, social services and secondary care providers, according to patient needs and appropriate care pathways.
* To maintain communication with all healthcare professionals and outside agencies to ensure seamless, patient-centred care.
* To ensure effective communication so that the patient receives the appropriate level of care and wherever possible avoid unnecessary hospital admission.
* To take an active role in maintaining expert communication with all patients, carers and relatives, which meets their individual needs.
* To deal with complaints in a calm and courteous manner, ensuring that, wherever possible, complaints are dealt with efficiently and satisfactorily on a local level.
* To communicate effectively in demanding and challenging situations, where there may also be hostility.

Professional

* To input to and extract information from the Practices clinical IT system, EMIS.
* To work collaboratively and collegiately with other members of the healthcare team such as the Community Nursing Team, Community Mental Health Team, care home staff, the acute ambulance and hospital trusts etc.
* To execute mandatory reporting duties, with particular regards to child and adult safeguarding.
* To participate and contribute to clinical meetings.
* To identify clinical and medicolegal risks within personal practice and organisational service provision, and to highlight these to the Partners.
* To work within the requirements of Data Protection legislation as it applies to patient records.
* To engage in self-appraisal and utilise reflective practice to enhance and develop personal practice.
* To maintain Continued Professional Development to a high standard.
* To maintain current professional registration.
* To maintain all mandatory training requirements with the necessary deadlines.
* To act at all times in such a manner as to promote confidence and public trust, and to uphold the reputation of Pembroke Medical Group and the individuals profession.

Organisational

* To operate as an effective member of the Practice team
* To provide information for the evaluation the role and for performance management purposes as requested.
* To assist in clinical audits in-line with the Practices clinical audit plan, and to participate as research as requested.
* To support the development of the Acute Day Team at the Practice.
* To contribute to the analysis of information to inform future development of the service.
* To assist in marketing and promoting the role and organisation within the health community both locally and nationally as required.

Person Specification


Experience

* Experience of the Advanced Paramedic Practitioner / ECP role within Primary or Secondary Care.
* Excellent organisational skills.
* Ability to manage and prioritise workload.
* Basic IT skills in word processing and e-mail.
* Experience in using electronic documentation and patient record systems.
* Proven ability to work autonomously and as part of a team.
* Able to work autonomously, using own clinical judgment to form a diagnosis.
* Skills for managing minor injuries.
* Skills of multi-system assessment.
* Knowledge of medical model of documentation.
* Able to give appropriate telephone advice.
* Understanding of health education/promotion.
* Able to give relevant safety-netting advice.
* Experience of following referral pathways.
* Experience of triage / initial assessment.
* Current knowledge of government targets and initiatives and N.I.C.E. guidelines and their impact on clinical practice.
* Understanding of the HCPC Code of Professional Conduct.
* Knowledge of child and adult safeguarding procedures.
* Understanding of good clinical governance and its significance and impact upon practice.
* Able to professionally challenge differing opinions as a patient advocate where appropriate.
* Evidence of experience / competency in the form of a professional portfolio.
* Experience as a practitioner in an Emergency Department or Urgent Care Centre / Minor Injuries Unit.
* Qualified as a non-medical prescriber.
* Able to record and interpret an electrocardiogram.
* Experience of performing a social assessment.
* Experience of performing brief cognitive assessments.
* Experience of clinical audit.
* Experience of research projects.
* Evidence of leadership skills.

Qualifications

* Relevant BSc or MSc
* Currently registered as a Paramedic (HCPC) or Nurse (NMC)
* Evidence of continued professional development.
* ECP / ENP Minor Injuries and Minor Illness Course
* Completed X-ray/IRMER course.
* Teaching and assessing qualification.
* Completed an NHS Clinical Leadership Programme or equivalent
