An exciting opportunity has arisen within the One Coventry Integrated Team for Band 2 Support Workers to join a community-based multidisciplinary team, providing reablement support for patients presenting with an acute deterioration in health and/or functional ability.
The aim of the service is to prevent and reduce avoidable hospital admissions and facilitate early hospital discharges by supporting patients through a reablement process to maximise independence. The service also identifies when independence is not achievable and signposts accordingly to ensure appropriate long-term support.
Please note that a full valid UK driving license and access to a vehicle for business use are required for this role.

Successful applicants will be required to provide an immunisation and vaccination report from their current UK occupational health provider or GP.
As a valued team member, your development will be supported through a robust induction programme, appraisals, one-to-one meetings, regular in-service training, and supervision. You will work from a multi-professional base, providing opportunities for joint working with Advanced Clinical Practitioners, Nurses, Care Coordinators, Physiotherapists, Occupational Therapists, Social Workers, Support Workers, and other health and social care professionals.
Part-time hours are available, working a mixture of shifts:
* Early: 08:00 – 14:00
* Late: 16:00 – 22:00
Up to 24 hours per week (4 days out of 7).
