This post covers the Sherwood locality, based in Rainworth. The division operates under three Units of Management, offering a broad portfolio of children’s, adults’, and specialist services across a large geographical area. We provide various clinical roles within supportive teams committed to delivering high-quality care.

Discharge to Assess (home first) is a new service in Nottinghamshire, designed to facilitate earlier patient discharge once medically fit, using advanced systems and technology for efficient reablement care. It centers on patient involvement and aims to improve health and wellbeing outcomes.
The team supports individuals in increasing independence through assessments, interventions, reablement, rehabilitation, and management of long-term conditions, working collaboratively with patients and carers. This service is vital in transforming community health services, ensuring high-quality care tailored to patient needs.
The service operates 7 days a week, from 08:00 to 20:00.
