Occupational Therapist - Discharge to Assess Services and Intermediate Care at Home
The Discharge to Assess team is an innovative service aiming to facilitate timely discharges for residents of Stockport and surrounding areas. Working as part of a multi-disciplinary team, the post holder will facilitate timely discharges from acute hospital sites and provide assessment and support planning to meet individual needs in the community. Responsibilities include providing and/or arranging appropriate support interventions in collaboration with colleagues across sectors.

We are looking for motivated, forward-thinking clinicians with a specialist interest in frailty, capable of working alongside an integrated multidisciplinary team to enhance patient flow and journey quality. Candidates should possess excellent communication, organizational, and leadership skills, with the ability to work independently and adapt to service developments.
You will be part of an integrated therapy team committed to staff development through CPD opportunities, supervision, and regular training. The role involves weekend working, including hours until 9 pm, supported by a teaching and competency program.
