Job Overview
We are looking for a skilled individual who can act as Care Manager/ Co-ordinator for our client and his long-standing care team. Post duties will include preparation of rotas for our client and liaising with the care team on holidays/ care planning/ managing sickness and arranging cover if required.
This post can be mainly carried out remotely (e.g. working from home), however we would expect face to face contact at least once a month with the care team for general care review meetings to be conducted.
The successful candidate must have good leadership experience within a health and social care setting. Excellent communication skills with the ability to build working relationships with the team is also essential in addition to a positive and proactive approach to problem-solving and decision making.
Key responsibilities include:
* Liaising with our client when preparing rotas to ensure sufficient cover is in place on a day-to-day basis and also when our client is taking holidays abroad and/or in the UK
* Implementing and monitoring electronic systems for care planning, staff scheduling and record-keeping
* Acting as a first point of contact for staff to inform of any sickness/ absences and liaising with the team to arrange care cover
* Participating in and facilitating care review meetings and supervisions for the care team
