Grade: Level 7
Salary: £31,022 - £33,699
Hours: 37 – Full Time

This is a Fixed Term post for 3 years.

Join our Active Communities Team and help transform lives across Stoke on Trent.

We're looking for passionate, skilled Health & Wellbeing Coaches to deliver high quality physical activity and behaviour change programmes that support residents to improve their health, confidence and long term wellbeing.

In this varied and rewarding role, you'll design and deliver specialist sessions for people with long term conditions, including cardiac rehab, diabetes, Parkinson's, frailty and weight management, using evidence based practice and motivational coaching techniques.

You'll carry out health assessments, support participants through their journey and build strong relationships with GP practices, pharmacies and community partners.

You'll need a GP Exercise Referral qualification, at least one Level 4 specialist exercise qualification, strong communication skills and experience of delivering safe, effective activity in community settings.

A commitment to behaviour change, excellent organisation and the ability to work independently across multiple venues are essential.
