Overview
We are recruiting for a Band 5 Development post within the NHS@Home Respiratory team. The successful candidate will receive a robust workplan and competency framework to support progression from band 5 to band 6. The candidate will receive weekly allocated study time and supernumerary shifts to develop skills across the respiratory service. This is open to both Nurse and Physiotherapist colleagues.
Responsibilities
* Expert clinical assessment, advice and treatment for people with acute respiratory needs within the community setting.
* Provide specialist assessment for patients on the Acute Respiratory at NHS@Home Pathway.
* To perform specialist respiratory assessments in a variety of settings by undertaking a holistic approach and following local and national guidelines and protocols.
* Work with service users and their carers towards shared goals providing advice and support, signposting to other community services and organisations to ensure person centred care and promoting physical, psychological and social wellbeing.
* Provide education and advice to enable self-management and prevent unnecessary hospital admissions.
* To work with patients as part of an integrated team and in partnership with other health care providers.
* Be a proactive member of the multi-disciplinary team and have a significant role in the ongoing service development, delivery and evaluation of the Respiratory@Home service across BNSSG.
