Community Psychiatric Nurse

This role is suitable for Registered Nurses who have successfully completed preceptorship and have registered as an approved NMC mentor. The post holder will deliver, or advise on the delivery of, high quality safe nursing care, ensuring consistently high standards of practice and clinical excellence which comply with the Nursing and Midwifery Council (NMC) code of professional conduct.

The Post Holder will provide community-based assessment, treatment, and care for people, aged 18-65, who have a history of offending, and who have mental health needs.

The post holder will aim to provide a flexible, responsive and pro-active programme of care and support for the service users and their carers. you will be required to develop and maintain links across a wide variety of statutory and non-statutory organisations such as Probation, MAPPA, social services, Jigsaw, Hostels and other voluntary and non-voluntary services. You will also be involved in specialist mental health assessments and the application of evidence based psychosocial interventions.


Main duties of the job

An exciting opportunity has arisen for a Community Psychiatric Nurse to join the Southwark Community Forensic Team. We are looking to find a dynamic, creative, forward thinking and hard-working nurse to work within our friendly team.

The Post Holder will work as a member of a borough based Forensic Community Team and will manage a reasonable caseload of clients with offending history and mental health problems.

The post holder will aim to provide a flexible, responsive and pro-active programme of care and support for the service users and their carers. you will be required to develop and maintain links across a wide variety of statutory and non-statutory organisations such as Probation, MAPPA, social services, Jigsaw, Hostels and other voluntary and non-voluntary services. You will also be involved in specialist mental health assessments and the application of evidence based psychosocial interventions.

Job responsibilities

To be reviewed in conjunction with the post holder on a regularbasis. You may be required to work at other locations within the Trust asdetermined by the duties of your post.

You may be required to undertake any other duties at the request of the line manager which are commensurate with the role, including project work, internal job rotation and absence cover. Person Specification


Qualifications

* Relevant professional qualification- Mental Health Nursing, Social worker, Occupational Therapist with affiliated professional registration NMC, HCPC


Experience

* Experience of Interagency of working
* Experience of managing a caseload
* Significant relevant clinical experience
* Experience of working with diverse groups of patients and staff
* Experience of working in the community
* Experience of supporting practice development in clinical areas


Skills and Abilities

* Knowledge of CPA, Community Care Act, Carers Act, Mental Health Act and other relevant legislation as application to practice
* Skills in mental health and risk assessments
* Understanding of Care Coordination and CPA
* Skills in being responsive to service user's and carers' needs
* Understanding of recovery in Mental Health
* Problem solving, ability to identify problems, review options and make decision
* Skilled in supervision of others, and in providing critical and constructive feedback
* Knowledge of clinical governance and audit


Other Requirments

* Empathy for service users including individuals who have experienced mental health problems.
* Ability to fulfil all the requirements of the role (including physical)
* Loyal and able to advocate for and show pride in the work area and organization and demonstrates values of trust and expectations


Disclosure and Barring Service Check

This post is subject to the Rehabilitation of Offenders Act (Exceptions Order) 1975 and as such it will be necessary for a submission for Disclosure to be made to the Disclosure and Barring Service (formerly known as CRB) to check for any previous criminal convictions.
