Job summary
The Hastings Primary CareNetwork is in search of a compassionate and committed Frailty Specialist CareCoordinator to join our rapidly expanding team. In this role, the Frailty CareCoordinator will play a crucial administrative role, collaborating withpractices and the broader community care team to ensure that frail patientsreceive appropriate interventions aimed at assisting them in maintaining asmuch independence as possible.
This entails coordinatingvarious clinician and social care services, as well as assisting individualpatients in identifying and accessing support services provided by thevoluntary sector or healthcare providers.
Main duties of the job
1. Reviewing Paramedic home visits to try to proactivelyidentify frailty and intervene before crisis
2. Arrange multidisciplinary teams between Primary& Secondary Care, Hospice and Care Home staff etc to collaborate and optimisepatient care.
3. Liaise with the frailty nurses based inA&E to identify issues and barriers, being the main point of contact.
4. Support discharges from hospital, ultimatelycreating robust processes and pathways between Primary, Secondary and CommunityCare.
5. Improve data quality coding of frail and houseboundpatients in the GP medical system.
6. Visit each of the eight PCN practices toreview their individual processes,toassist in standardising patient care
7. Work closely with the Clinical Leads toensure smooth working and training between the Care Homes and themselves.
8. Facilitate referrals to health and care services,being the main point of contact.
9. Help improve patient access to GPs.
10. Ensure practices are aware of how to keep upto date with the most useful information for all services in the Summary CareRecord

11. Strong interpersonal and communication skills to engage effectively with patients, families, and healthcare teams
12. Excellent organisational skills with the ability to manage multiple tasks and prioritise effectively
13. Proficiency in using electronic medical record systems and other relevant software (training available)
14. Compassionate and empathetic approach towards patients and their families
15. Ability to work independently and as part of a collaborative team with ability to work under time pressure
16. Proactive work style, must be able to solve problems autonomously
17. Must be able to take responsibility for the remit of tasks and progress, unsupported or proactively seek support from the available resources whenever necessary
18. Manage and prioritise multiple tasks

19. Previous experience of working in General Practice
20. An understanding of frailty and the issues involved
21. Experience in Care Coordination
22. Competent in using clinical systems (EMIS) and Office applications such as Excel
23. Knowledge of local frailty support services
