Join Us as a Social Worker

Directorate of Adult Social Care & Health

Are you passionate about making a real difference in the lives of vulnerable adults? Do you believe in empowering people to live independently, with dignity and respect? If so, we invite you to join our dedicated team as a Social Worker, in our Hospital Social Work Team, supporting with hospital discharges across multiple sites both within the East Riding of Yorkshire and within our bordering authorities and occasion further afield. The Hospital Social Work Team has also implemented some exciting changes and have remodelled the way we work to support the principles of Home First Model- supporting a ‘discharge to recover and then assess’ approach which enables people the time and space to recover in the place they call home before we consider and assess for any longer-term care options.

About the Role

As a Social Worker, you will:

* Plan and deliver support to help adults remain well and independent.
* Manage a caseload, offering expert advice and high-quality assessments.
* Chair meetings, contribute to case conferences, and produce detailed reports.
* Model good practice, build capacity in others, and contribute to service improvement.
* Apply legal frameworks and guidance in complex situations, including mental capacity, court work, and safeguarding.
* Supervise staff and support the development of colleagues and students.

What We’re Looking For

Essential:

* Qualified Social Worker (DipSW, CQSW, Social Work degree, or equivalent).
* Experience in adult social work settings.
* Registration with Social Work England and understanding of professional standards.
* Strong knowledge of relevant legislation (Care Act 2014, Mental Capacity Act 2005, Health & Care Act 2022 etc.).
* Excellent communication skills and the ability to work with diverse individuals and teams.
* Commitment to confidentiality, data protection, and continuous professional development.
* Full driving license and suitability to work with vulnerable adults.
* Have a strength-based approach with a focus to support people to achieve and maximise their independence
