Overview
The post holder will lead and develop community services across community teams, clusters and secondary care, supporting the Hospital@home service and community discharge teams. As a key member of the Primary and Community Care Group leadership team the focus will be on delivering integrated person‑centred care and embedding the transformation agenda.
Responsibilities
* Operationalise community transformation plans, meeting organisational quality, performance and financial targets.
* Implement strategic direction at an operational level for Hospital@Home and Discharge Service.
* Contribute to service redesign, manage services and guide the Directorate Manager.
* Collaborate with integrated teams to roll out best practice and seamless pathways.
* Ensure clear referral system and monitor effectiveness via performance framework.
* Maintain high standards of health and care service delivery and innovate to improve quality.
Qualifications & Knowledge
* Master’s Level qualification or equivalent relevant skills, knowledge and experience.
* Knowledge of the political context and constraints at local and national level.
* Comprehensive understanding of governance principles and systems.
Desirable
* Postgraduate management qualification.

* Operational management of staff, performance, service development and financial management.
* Strategic contribution to plans and policies.
* Effective partnership working with stakeholders.
* Leading and establishing effective teams.

* Working in a political environment and building relationships with local elected members.

* Welsh language speaking/listening skills at Level 3 or above.
Welsh Skills Desirable
Welsh is desirable. Speaking & listening skills at Level 3 (equivalent to CEFR B2) or above will be considered an advantage during short‑listing.

£65,424 to £76,021 per annum (Band 8b)
