30 hours per week Actual Salary: £22,578 per annum About the role: Our Home From Hospital Team aim to support the local health & social care systems, by facilitating timely patient discharges and preventing unnecessary readmissions by connecting customers to community services that in turn reduce the demand on statutory services. A key part of this role is to identify patients and carers requiring practical, non-medical support to ensure a safe and timely transition home following a stay in hospital. You will carry out a mixture of advice, guidance, signposting and practical tasks such as assessing & arranging delivery of aids and adaptations, arranging installation of key safes or Carelink systems. You will also advise and support customers with tasks such as claiming benefits, debt management and housing issues that promote independence and improve health and wellbeing. We support Weston General Hospital and other local hospitals where residents are returning home to North Somerset. On a rota basis you will visit the different hospitals we work with, therefore you will need to be able to travel out of area independently when needed. About you: Having experience of working in a support, health, or care environment, you will be able to demonstrate your excellent customer support and customer service skills from working with a range of people. You will also need great communication skills, both written and interpersonal. Please note: this is not a domiciliary/personal care role. On occasion, to meet the needs of our customers, you may need to work outside of your traditional hours. Due to the nature of this role, you will need to be able to drive with access to your own vehicle or have access to alternative transport. The successful candidate will also be subject to a satisfactory Enhanced DBS check and hospital security check.
