The team provides specialist assessment and support for people presenting with possible onset of dementia and/or a diagnosis of dementia. You will work as part of a multi-disciplinary team offering person-centred support to improve the quality of life for people living with dementia, their families, and carers, with a commitment to living well with dementia.
We are offering an exciting opportunity for a Community Support Worker passionate about working with people living with dementia to join our Dementia Home Treatment Team. You will need excellent communication skills, be enthusiastic, and flexible in your approach.
As part of our service, you will provide signposting and support through planned interventions to people with dementia in the community, following an agreed care plan and under the guidance of a qualified practitioner. The role offers variety and scope to demonstrate and develop your skills. You will work closely with and be supported by our multi-disciplinary team, including admin staff, fellow support workers, psychologists, consultant psychiatrists, nurses, and occupational therapists. Additionally, you will collaborate with our wider health and social care community, including general practice, care homes, social care, admiral nurses, and third-sector services such as the Alzheimer's Society.
Key responsibilities include:
1. Ensuring high-quality care delivery within the Dementia Home Treatment Team and maintaining high personal standards in clinical practice.
2. Providing care in line with the National Dementia Strategy, NICE guidance, and relevant legislation, including the Mental Health Act 1983 (revised 2007) and the Mental Capacity Act 2005.
3. Participating in work prioritization and caseload management with support and supervision.
4. Contributing to risk assessments and management plans, supporting positive risk-taking to enhance quality of life, well-being, and independence.
5. Delivering non-pharmacological interventions based on specific assessments to address unmet needs and maximize individual abilities.
6. Providing signposting and support to individuals with dementia through planned interventions, following care plans and under supervision.
7. Working flexibly across the countywide service based on skill and service needs.
