Occupational Therapist Specialist (Mental Health Liaison Team)

The post holder will be a key member of the multi-disciplinary Mental Health Liaison Team, providing assessment and intervention for patients with mental health needs. The role involves community assessments, collaboration with various health and social care professionals, and discharge planning to promote patient well-being and prevent unnecessary hospital admissions.
Welsh language skills are not essential but may be advantageous, as the role involves contact with Welsh-speaking patients. Welsh language courses are available through CTMUHB.
The post holder will work across hospital and community settings, including care homes, and will be responsible for delivering on performance indicators such as reducing length of stay and improving independence. They will assess patients' ability for safe discharge, liaise with families and carers, and work within a multi-agency team to ensure effective care and discharge planning.
Main duties of the job
1. Autonomously apply the Occupational Therapy process in various settings, including community and lone working in care homes.
2. Manage a caseload of patients referred to the Mental Health Liaison Team, conducting specialist mental health assessments.
3. Use evidence-based, patient-centered approaches to assess, plan, implement, and evaluate interventions in hospital and community settings.
4. Collaborate with multidisciplinary teams, including hospital staff, care managers, and external agencies, to deliver assessments and discharge planning.
5. Communicate effectively with patients, families, carers, and staff, using appropriate verbal and non-verbal techniques, especially in challenging situations or where barriers exist.

Person Specification
Qualifications/Knowledge
* Degree/Diploma in Occupational Therapy
* Post-graduate training in fieldwork education
* HCPC registration
* Commitment to CPD
* Membership of a professional body
Experience
* Post-graduate clinical experience, including mental health settings
* Experience in service delivery, training, supervision, and research
Knowledge
* Broad understanding of mental health conditions and discharge factors
* Knowledge of clinical governance, partnership working, and professional standards
Skills and Other Attributes
* Effective communication, interpersonal, and teaching skills
* Autonomous working and organizational skills
* Ability to work flexibly across settings and with diverse populations
