There is an exciting opportunity available - We are looking to appoint a Grade 9 Specialist Autism social worker, based within the High Needs Team in Hull. If you are inspired by a challenge and want to make a lasting difference, to support adults who have been assessed as having Care Act Needs and may have a diagnosis of Learning Disability, Autism or Mental Health and or physical health support; we look forward to hearing from you. You will work as part of an integrated multi-disciplinary team to provide high quality social work practice. You will ensure individuals have a solution focussed, strength-based plan towards achieving their goals and maintaining their independence.
The team you will be part of follows a Multi-Disciplinary approach, working alongside health professionals, probation, criminal justice, housing, and incorporates commissioning bespoke provisions for persons with complex needs; to deliver the best possible outcomes for Adults living in Hull.
