The Adult Social Care Transitions Team (14-25) provides social care assessment, case management, and safeguarding support across Staffordshire, working alongside young people living with disabilities and additional needs, including neurodiversity and mental health challenges. The team collaborates with SEND and education provisions to support transitions, involving Social Workers, Social Care Practitioners, children’s social workers, SEND workers, and health and education professionals. The team's focus is to support young people in experiencing a successful transition into adult social care, helping to prevent abrupt changes for those they support.
This team is dedicated to ensuring the best possible transition for young people. The Senior Social Worker role is vital in supporting the workforce and ensuring strength-based practice. The role is based in Stafford town centre (Staffordshire Place 1), requiring attendance at the office at least twice a week, with the option for home working on other days.
Main Responsibilities
1. Provide advice and guidance to practitioners to ensure high-quality service delivery.
2. Carry out quality assurance on assessments and provide strength-based feedback.
3. Provide professional supervision to approximately 8 staff members.
4. Support duty management on a rota to ensure team functions effectively.
5. Manage a small, complex caseload, possibly as a keyworker or joint worker with team practitioners.
6. Support court work as needed.
7. Facilitate quality circles to promote best practice within the team.
8. Chair complex MDT and best interests meetings.
9. Ensure key performance indicators are met.
The Ideal Candidate
1. Be an experienced social worker with a track record of managing complex cases.
2. Demonstrate post-qualification learning.
3. Have understanding and experience of Deprivation of Liberty Safeguards.
4. Possess relevant safeguarding training and experience.
5. Have experience working with transitions and supporting young people moving from childhood to adulthood.
6. Experience in supervising practitioners.
