Social Worker – Community Mental Health Team Location: Central Clinic, Orchard Street, Swansea
Contract: Full-time (37 hours per week)
Hybrid working: Combination of office, home-based, and community visits
Description
The City and County of Swansea are seeking a dedicated and experienced Social Worker to join the Community Mental Health Team (CMHT). This is an exciting opportunity to make a real difference in the lives of individuals experiencing serious mental health challenges, helping them to live as independently and meaningfully as possible within the community.
You’ll be part of a multi-disciplinary, recovery-focused team, working collaboratively with health professionals, carers, and families to provide integrated and person-centred care.
Responsibilities
Undertake statutory social work duties in line with the Social Services and Well-being (Wales) Act 2014 and Mental Health Act 1983.
Assess the needs and circumstances of individuals and families, developing tailored care and support plans.
Act as Care Co-ordinator, ensuring care packages are well planned, monitored, and evaluated.
Attend Mental Health Tribunals and contribute to care reviews and planning meetings.
Carry out risk assessments and take appropriate steps to safeguard vulnerable individuals.
Work collaboratively with internal and external agencies to promote recovery and maximise service user participation.
Maintain accurate, timely, and confidential case records.Requirements
Qualified Social Worker registered with Social Care Wales.
Proven experience in mental health social work, preferably within a community or multi-agency setting.
A valid driving licence and ability to travel as part of the role.
Knowledge of WCCIS (Welsh Community Care Information System) is advantageous.How to Apply
