Main duties of the job

In this vital role, you will be responsible for the comprehensive assessment, planning, delivery, and evaluation of nursing care for your patients. Your key duties will include administering medications and treatments, monitoring patient conditions, and accurately maintaining patient records.

A central part of your role involves working collaboratively within our multi-disciplinary team to ensure seamless, coordinated care. You will act as a key advocate for patients and their families, providing essential emotional support, clear information, and expert health education. Furthermore, you will supervise and support junior staff, including student nurses and healthcare assistants, contributing to our culture of mentorship and continuous improvement in patient safety and clinical standards.

Job responsibilities

Community Staff Nurse

Our dynamic District Nursing Team is looking for a dedicated and enthusiastic Band 5 Nurse to join us in delivering exceptional, patient-centred care within the community. This is your chance to step away from the ward and into the heart of our local neighbourhoods, making a real difference in patients' lives where it matters most in their own homes.

In this rewarding role, you will be an integral part of our friendly team, working under the guidance of a qualified District Nurse to provide high-quality nursing care to a diverse patient population. You will manage a varied caseload, from supporting patients with long-term conditions and complex wound management to delivering end-of-life care with compassion and dignity.

Person Specification


Qualifications

* Is a qualified Registered Nurse (Adult) with a valid NMC pin.
* Is a car driver with a full, valid UK licence.*
* Has a passion for community nursing and a desire to empower patients.
* Is an excellent communicator with strong clinical assessment skills.
* Is adaptable, resilient, and can work effectively both independently and as part of a team.
