The CareNavigator plays a vital frontline role within the practice, supporting patients to access the most appropriate healthcare professional or service based on their needs. The post holder will provide high-quality reception and administrative support, ensuring patients receive a courteous, efficient, and confidential service at all times.
Working as part of the multidisciplinary team, the Care Navigator will help improve access to care, support effective service delivery, and contribute to the smooth day-to-day running of the practice.
Core Responsibilities
* Process and effectively signpost patients to the appropriate healthcare professional depending on the presenting condition.
* Answer incoming phone calls, transferring calls or dealing with the caller’s requests appropriately.
* Process patient requests for appointments.
* Initiate contact with and respond to requests from patients, team members and external agencies.
* Enter read-code data via Templates on EMIS Web.
* Photocopy documentation as required.
* Data entry of new and temporary registrations and relevant patient information.
* Input data into patients’ healthcare records as necessary.
* Direct requests for information, e.g. SAR, insurance/solicitors letters and DVLA forms, to the administrative team.
* Direct complaints as appropriate.
* Manage all queries efficiently.
* Carry out system searches as requested.
* Maintain a clean, tidy, effective working area at all times.
* Support all clinical staff with general tasks as requested.
* Provide administrative cover during periods of staff absence.
Experience
* Experience of working in a primary care environment.
* Experience of working with the general public.
* Experience of working in a healthcare setting.
Qualifications
* Educated to GCSE level or equivalent.
* Active signposting or Care Navigator training qualification.
* Healthcare qualification (level 2) or working towards gaining equivalent level.
