Are you a motivated GP looking to develop your career within an innovative Primary Care Network (PCN) setting?
We are seeking a dynamic salaried GP to join our multi-disciplinary team as part of a wider PCN, focusing on improving outcomes for our most vulnerable patients through managing frailty and preventing unnecessary hospital admissions. This position is for 6-8 sessions per week.
This role offers the opportunity to work across several PCN practices, delivering proactive, holistic care for patients with complex needs, particularly those living with frailty. You will be a key player in a growing ARRS team, collaborating closely with multidisciplinary colleagues including ECPs, pharmacists, and care coordinators.
Main duties of the job
Your work will actively contribute to admission avoidance strategies through population health data analysis and tailored patient interventions, ensuring efficient, high-quality care provision. You will have the autonomy to shape your clinical approach while benefiting from the supportive structure of the PCN.

About us
The Dengie and South Woodham Ferrers Primary Care Network is a collaboration of seven GP practices, serving approximately 45,000 patients from the Dengie, Bicknacre, Burnham-on-Crouch and South Woodham Ferrers area. Person Specification
Qualifications
* Fully qualified GP (CCT within the last 2 years and no substantive GP post held) (experience with frailty management beneficial but not mandatory).
Experience
* Passionate about population health management and proactive care for frail and complex patients.
* Confident using data-driven approaches to guide clinical decision-making.
* Excellent communicator with a team-oriented mindset.
* Eager to develop professionally within a supportive and innovative network.
