The Pall Mall Surgery is looking to welcome a qualified and experienced clinical pharmacist into our busy practice, to play a full and active role within our multi‑disciplinary team.
The role is offered on a part‑time basis for 20–23 hours per week on Tuesday, Wednesday and Thursday.
Main Duties
 * Clinically assess and treat patients, using expert knowledge of medicines, for specific disease areas.
 * Provide specialist expertise in the use of medicines and medicine information (e.g. side‑effects, dosage).
 * Provide pharmaceutical medication review and health‑education advice to patients, by face‑to‑face, telephone or notes‑based review, as agreed with the practice.
 * Provide leadership on person‑centred medicines optimisation and quality improvement.
 * Process specialist treatment letters and hospital medication discharge letters.
 * Act as a key contact for all medication related queries.
 * Support improvements in clinical care through practice‑based audits and implementing change.
 * Review daily pathology results and act on results.
 * Undertake minor ailments triage and direct patients based on clinical need.
 * Capture relevant patient health information at all consultations, including contributing to the quality outcomes framework.
 * Establish ongoing professional relationships with individual patients.
 * Be part of a professional clinical network and have access to appropriate clinical supervision.
 * Manage the repeat prescribing re‑authorisation process by reviewing requests for repeat prescriptions and medicines reaching review dates.
 * Review prescribing against local and national targets, and engage with the wider prescribing team to rectify any issues.
 * Carry out structured medicines‑optimisation reviews in care homes.

The Pall Mall Surgery is a large, modern GMS training practice operating from a contemporary health centre in Leigh On Sea. In 2022 it was voted the best place to live in Essex. The practice supports 21,500 patients from birth to end‑of‑life care and holds a CQC rating of Good in all areas.
Pay and Contract
Pay: £21.57 to £27.18 per hour (Variable Salary). Contract: Permanent. Working pattern: Part‑time.

Qualifications (Essential)
 * Completion of a degree in pharmacy and registration with the General Pharmaceutical Council.
 * Post‑qualification experience in pharmacy (hospital, primary care or community), or working toward obtaining this.
Experience (Essential)
 * Awareness of the breadth of common acute and long‑term conditions likely to be seen in a general medical practice.
 * Demonstrated general pharmaceutical skills and knowledge in core areas.
 * Ability to plan, manage, monitor, advise and review general care plans for patients with long‑term conditions.
 * Accountability for delivering professional expertise and direct service provision as an individual.
 * Ability to recognise priorities when problem‑solving and identify deviations from normal patterns, referring to seniors or GPs with confidence when appropriate.
 * Adherence to legal, ethical, professional and organisational policies, procedures and codes of conduct.
 * Ability to assist patients in decisions about prescribed medicines and support adherence as per NICE guidance.
 * Non‑judgemental attitude with flexibility and adaptability.
 * Self‑motivated with excellent communication skills.
 * Understanding of patient confidentiality.
