Supported Assistant Practitioner (Scale F) / Assistant Practitioner (Scale G)

Pay: £28,598 to £33,699 per annum (Scale F/G)
Hours: 37 per week
Location: CEMP North Walsham, Norfolk & Norwich University

We are looking to appoint either a Supported Assistant Practitioner (Scale F) or an experienced Assistant Practitioner (Scale G). Candidates must provide a CV and Supporting Statement setting out how they meet the essential and desirable criteria of each role. Job Summary
You will support the communities of Norfolk using your previous Health or Social Care experience as part of a full‑time, hybrid role. Working closely with the Norfolk and Community Care (NCH&C) and NHS colleagues, you will help ensure safe and timely hospital discharge for those in community and acute hospitals across the Central Area of Norfolk. This is a new, developing team committed to supporting people to return home, access reablement opportunities and plan their ongoing care and support needs once discharged.

Programme
As a Supported Assistant Practitioner (Scale F) you will join a 9‑month bespoke support programme, enabling progression to an Assistant Practitioner role (Scale G) subject to satisfactory programme completion.

Key Responsibilities and Qualifications
• Experience in the Health or Social Care sector
• Good standard of education and excellent IT and organisational skills
• Ability to work independently or as part of a team in a fast‑paced, responsive environment
• Positive can‑do attitude and strong analytical and listening skills
• Flexibility and adaptability to new ways of working and commitment to supporting people to leave hospital safely
