About the Role
Our Living Well Centre offers a programme of activities and interventions designed to support patients and carers physically, emotionally, socially and spiritually. As a Living Well Assistant, you will play a central role in ensuring the Living Well Centre runs smoothly and that every person feels welcomed, supported, and empowered.
Responsibilities
* Supporting patients and carers during group activities and individual interventions.
* Liaising with the multi‑disciplinary team to ensure co‑ordinated, high‑quality care.
* Working autonomously, where appropriate, to meet individual patient and carer needs.
* Providing empathetic, clear communication and telephone support.
* Engaging with our community to raise awareness of the Living Well Centre.
* Conducting occasional home‑based assessments as needed.
* Ensuring a warm, inclusive environment for everyone who attends.
* Providing administrative support including clinical record‑keeping and referral processing.
* Contributing to service development, projects and audits.
Requirements
* Strong communication and interpersonal skills.
* Ability to work both autonomously and as part of a multi‑disciplinary team.
* Compassion, emotional resilience and a person‑centred approach.
* A flexible, proactive and positive attitude.
* Good organisational, IT and time‑management skills.
* A full driving licence and access to a car for business use.
* Willingness to work towards a Level 3 Health & Social Care qualification (if not already held).
* Experience as a Health Care Assistant, Therapy Assistant or similar.
* Experience supporting people in a group setting.
* Experience in palliative care, community development or working with volunteers would be an advantage.
