Job Title: Hospital Social Worker
Location: Huddersfield Hospital
Hourly Rate: £35 per hour

Our hospital's social work team plays a crucial role in ensuring the well-being of patients and their families by addressing psycho social, emotional, and practical needs throughout their hospital stay.
Position Overview: We are seeking a dedicated and experienced Hospital Social Worker to join our team on an hourly basis. The Hospital Social Worker will work collaboratively with patients, families, health care professionals, and community agencies to provide support, advocacy, and intervention to address psycho social issues impacting patient's health and recovery.
Key Responsibilities:
1. Conduct comprehensive psycho social assessments of patients and their families to identify needs, strengths, and preferences.
2. Provide emotional support, counselling, and crisis intervention to patients and families facing challenging medical diagnoses, treatment decisions, or end-of-life care.
3. Collaborate with health care teams to develop and implement individualised care plans that address patient's psycho social needs, promote coping strategies, and enhance overall well-being.
4. Assist patients and families in navigating the health care system, accessing community resources, and addressing practical concerns related to discharge planning, housing, financial assistance, and transportation.
5. Advocate on behalf of patients and families to ensure that their voices are heard in care planning, decision-making, and discharge processes.
6. Facilitate communication and collaboration between patients, families, health care professionals, and community agencies to optimise continuity of care and support transitions across care settings.
7. Maintain accurate and confidential documentation of assessments, interventions, and referrals in accordance with hospital policies and regulatory requirements.
8. Participate in interdisciplinary team meetings, case conferences, and quality improvement initiatives to enhance patient care and service delivery.
Qualifications and Experience:
9. Degree in Social Work or equivalent qualification.
10. Registered with the appropriate regulatory body (e.g., Social Work England).
11. Previous experience working as a social worker in a health care setting, preferably in a hospital or medical centre.
12. Strong assessment, communication, and interpersonal skills.
13. Knowledge of medical terminology, health care systems, and psycho social issues impacting patients and families.
14. Ability to work collaboratively as part of a multidisciplinary team, managing a diverse caseload in a fast-paced environment.
15. Commitment to patient-centred care, ethical practice, and professional development.
