About the Role: The Royal Borough of Windsor and Maidenhead (RBWM) is seeking an experienced Social Worker to join its Hospital Team. You’ll play a vital role in supporting residents through safe, well‑coordinated hospital discharges, ensuring smooth transitions back into the community with the right support to maintain wellbeing and independence.
Role Overview
You will assess needs, coordinate care, and safeguard vulnerable adults, working closely with individuals, families, and partner agencies. Supported by a Senior Social Worker and Team Manager, you’ll promote dignity, choice, and empowerment while ensuring safe outcomes for residents.
Key Responsibilities
* Support adults to live independently and with dignity
* Carry out strengths‑based assessments and develop person‑centred care plans
* Safeguard adults from abuse, neglect, or exploitation (Care Act 2014 compliance)
* Liaise with families, carers, health professionals, and community services
* Promote empowerment, choice, and access to services
* Advocate for residents and coordinate holistic support
Candidate Requirements
* Degree in Social Work and registration with Social Work England
* Strong assessment and care planning skills
* Knowledge of adult safeguarding procedures and legislation
* Excellent communication and collaboration skills
* Resilience and professionalism in managing complex cases
Additional Requirements
* Disclosure and Barring Service (DBS) check required
* UK professional registration required
* Skilled Worker visa sponsorship available (subject to eligibility)
